{"id":1442,"date":"2015-05-11T11:51:43","date_gmt":"2015-05-11T18:51:43","guid":{"rendered":"https:\/\/phcprimarycare.org\/?p=1442"},"modified":"2015-05-18T12:05:49","modified_gmt":"2015-05-18T19:05:49","slug":"recent-developments-in-infectious-diseases-bugs-that-bug-us","status":"publish","type":"post","link":"https:\/\/phcprimarycare.org\/?p=1442","title":{"rendered":"Recent developments in infectious diseases\u2013bugs that bug us &#8211; Part I"},"content":{"rendered":"<p>I was recently asked to give a 15-minute rapid update on changes in infectious diseases that would be of interest both generally and for an audience of family physicians at the CAFP Clinical Update in San Francisco.<\/p>\n<p>The list of interesting items was long and I pared the brief discussion to a few items. The list:<\/p>\n<ul>\n<li>Antibiotics in severe sepsis<\/li>\n<li>Chikungunya<\/li>\n<li>Clostridium difficile<\/li>\n<li>Dengue<\/li>\n<li>Ebola<\/li>\n<li>EV-D68<\/li>\n<li>HCV<\/li>\n<li>HIV pre-exposure prophylaxis (PrEP)<\/li>\n<li>Influenza<\/li>\n<li>Measles<\/li>\n<li>Microbiome<\/li>\n<li>Moxifloxacin failure in short course TB<\/li>\n<li>Acute skin and skin structure infections (ASSSI)<\/li>\n<li>Carbapenem-resistant enterococcus<\/li>\n<li>Polyvalent pneumococcal vaccine<\/li>\n<\/ul>\n<p>I have culled the following points from the long part of the short talk:<\/p>\n<p><span style=\"text-decoration: underline;\">Antibiotics in sepsis<\/span> \u2013 no comment as this wasn\u2019t relevant to most PCPs.<\/p>\n<p><span style=\"text-decoration: underline;\">Chikungunya<\/span> \u2013 a mosquito-borne viral illness with over 6,600 cases in U.S. travelers to endemic areas of the Caribbean and even a few home grown cases in Florida \u2013 expect to see more in the southeastern U.S.<\/p>\n<p><span style=\"text-decoration: underline;\">Dengue<\/span> \u2013 getting close to a vaccine for this other mosquito-spread disease.<\/p>\n<p><span style=\"text-decoration: underline;\">EV-D68<\/span> \u2013 this echovirus causes respiratory illness, particularly in the young with asthma, but it has now also been linked with a polio-like motor paralysis from which recovery is slow and\/or incomplete.<\/p>\n<p><span style=\"text-decoration: underline;\">Influenza<\/span> \u2013 the vaccine was off-target this past influenza season. But we have learned that vaccination of a pregnant mother does extend protection to the newborn.<\/p>\n<p><span style=\"text-decoration: underline;\">Measles<\/span> \u2013 multistate outbreak linked to a source at Disneyland and spread among the unimmunized and in some cases the immunized. Perhaps Disneyland is not the Happiest Place on Earth for a while. 169 cases of measles reported to the CDC from January 1 to May 1, 2015, in 27 states. Wasn\u2019t measles elimination in the US announced in 2000?<\/p>\n<p><span style=\"text-decoration: underline;\">Microbiome<\/span> \u2013 fascinating science about how the personal microbiome can affect individual health. Why are 80% of antibiotics used in animal husbandry? It has long been known that antibiotics cause animals to gain weight! Get that!<\/p>\n<p><span style=\"text-decoration: underline;\">Moxifloxacin failure<\/span> in 4 month treatment course of smear-positive TB in combination with other drugs compared to standard treatment regimens.<\/p>\n<p><span style=\"text-decoration: underline;\">Carbapenem-resistant enterococcus<\/span> \u2013 spread by endoscope with some fatal outcomes. Makes me sad to be a Bruin alum.<\/p>\n<p><span style=\"text-decoration: underline;\">Pneumococcal conjugate vaccine<\/span> \u2013 recommended for those needing pneumonia prophylaxis.
